People managing chronic, lifelong conditions face the daily realities of long-term health management: complex routines, overwhelming information, and the weight of stigma. With limited time in consultations, patients often struggle to understand which small actions can truly improve their well-being. The challenge was to simplify health information and encourage meaningful, everyday actions that build towards healthier habits.
From overwhelming to doable
We used research, ideation and concept development to explore how health management could be made more understandable and motivating.
This included experimenting with physical and digital touch-points, multi-sensory experiences, and ways for patients to reflect on their actions.
